Prepare Form is identifying fields vertically instead of horizontally?

Hello,

Tried to use the Prepare Form function to convert a .pdf (actually only an image), that was emailed to me, into an editable form. Original doc is legal size, so had to rotate it. After rotating it, I selected Prepare Form. See image for results.

Anybody have any ideas?

See what happens if you don't first rotate it and rotate it only after the fields are automatically added.
