Form Field Layer templates

Hi 

When I try to create Reusable Templates, I dont have any option to select the type of the template.  When I try to use the template, it inserts the document which I used for creating the template vs Form Fields.  I have Acrobat Sign Prepaid Plan.  I have seen any option to enable this feature.   Based on the posts it is a enabled by default.
