Cannot set a document for signature to show signature, name and date

Hi,

I simply can't get Adobe Sign to work. If I try to put a Signature block on the page, it adds the words Signature and Email, which I don't want.  if I just drag on a Signature field, it doesn't show the person's name and date underneath the signature when they sign.

In short, I see this

But I want to see just this:

Use Email field to capture the email along with the Signature field to achieve your workflow.

Thanks Aadesh, but I think my question wasn't clear

1. I don't want an email field or an email address

2. I don't want the word Signature

I just want the signature to look exactly like this:

Which version of Adobe sign are you using? The enterprise version I am using exactly the way you wish it to behave.

In that case you can simply use the Signature field that will capture the Signature, name and date of the Signer.
