How to add Customer Sign on PDF

Hi - 

 

I Have Making Editable PDF, here required customer sign  (example- drag sign from his hand in MOBILE )  

how can i add there customer sign (which feild is need to add) 

 I attached my template as bellow image-

please sugest me
