How to complete annual updates on a signed form

We have a form that our families complete and then once a year they need to do an "update" and sign and date the bottom. Is there a way with Adobe Sign to do this easily?

If you want to capture this in a single agreement, then add 4 participants using the same email address, however there may be some practical issues with this approach.

For example the participant will receive another email asking to sign right after they sign the first time, so it would need to be clearly communicated as to what is epxected.
