html form integration

we need to simply integrate in the capability of the user to sign an online form submittal. is there someway to use adobe document esign to do the following?

1. user fills out form fields for application
2. user signs the application
3. user hits submit
4. all the data from the html form fields, plus their signature, is populated into an echosign/DC document.

is that possible or do we have to use an embedded echosign/DC document in our html?

Filling an online form can be done by creating a widget.
