A couple of years ago I created a form that was filled out with checkboxes & radio buttons, then sent for signatures. Example below:
Project Owner: {{ $ReqName}} | {{I0}} {{K0}} Audio | |||||
| ||||||
{{#ReqName=*RequiredName_es_:sender}}
Is this still possible to use, or do I need to drag&drop all the buttons?
Hi sarahm62404467,
Yes, you can do that with eSign services, but Form Central is discontinued.

Hi sarahm62404467,
You can add this form to Acrobat & prepare the PDF form, where you can add the fields & required buttons, further add action & properties.
Refer to this KB doc. for help Acrobat Help | Creating and distributing PDF forms.

In the past, I was able to create a form using word and these little codes in brackets that let me enter radio buttons, check boxes, comments and signature blocks (see initial post). When I loaded the file into Echosign, it would fill in the fields for me in a uniform manner - not trying to line up a hundred check boxes. Is this still possible or did that functionality go away when Adobe took over Echosign? My guess is that nobody at Adobe knew it was a possibility.
