Hmm... Let's start here.

Make sure your HTML5 button is grey and the SWF button is white.

Also make sure - if you need a SCORM project that proper reporting is enables as well.

That published folder is a SCO, which means you have turned on Reporting under Quiz Preferences.. You can only upload it to a LMS compliant with SCORM.

That is also the reason why it is zipped automatically by Captivate because that is also a SCORM requirement.
