I've just uploaded my test course to my companys LMS, going through and answered all the quiz questions and finished off the course by clicking an exit button I created on the last slide.
Once I then click 'Complete' on our LMS it get an error messaging stating "Not all fields have been entered" and "Course not completed".
Any guidence is appreciated , thanks guys.

From the screenshots I can detect that you are NOT using version 12 because Preferences are more limited there. However I would like to know the exact version you use, full version number which can be found under Help, About Captivate.
Your criterion for completion is very tough: only when quiz has Passed. Did you try this output on SCORM Cloud? Some LMSs have issues with a criterion in points, you could have used 100% percentage instead. I also wonder what you put in the Required field in the Reporting part of the Quiz Preferences. You didn't include that.
The Continue button on the score slide triggers the Pass/Fail actions, when reaching the last frame of that slide. Do not choose Go to Next slide as Pass action, keep 'Continue' and eventually move the pausing point a bit to the right to shorten the inactive part of the score slide.

11.8.254 is the version I'm using.
My boss wants users to have to finish all the quiz questions on the project and to get them all right before being able to finish the course. All questions have 2 trys. If then after that they have wrong answers, they must retake the quiz('Retake Quiz' button pops up on the quiz results slide and the 'continue' button is hidden if the user doesn't have all answers right)
I have changed the Success/Completion Criteria to Slide Views 100% as you suggested. Thank you.
I just had a look to see what the quiz required option was under, it was listed as optional. But I changed it to 'Pass required - User must pass this quiz to continue'. Still having the same issue.
I did have the continue button on the results slide set to 'go to next slide', so after your comment I changed it to continue. Thank you.
Unfortunately those changes have not fixed my issue.

I was not talking about slide views percentage because 100% there could also lead to problems because some LMSs consider a slide to be viewed only when it has reached the last slide frame which is not always the case if you have buttons.
No, I would leave the Required field to 'Optional'. The strict choices could lead to issues with some LMSs.

How did you 'hide' the Continue button, please? Did you do it by dragging it under the Retake button? That is the only correct way.
Another tip: allow Backwards movement. You don't need to add a Back button.
Strange request from your client, not very 'pedagogical'.
