David Power's book "The Essential Guide to Dreamweaver CS3 with CSS, Ajax, and PHP" from the Friends of Ed Adobe Learning Library (ISBN: 978-1-59059-859-7) is very good and I recommend this book to anyone. What makes it even better is that David pays attention to the small things that make a big difference and his book Covers Mac and Windows setups with Apache and IIS etc. (Personally I use MAMP/Pro on my Macbook with CS3 web premium. Works Great.
