Dynamic Radio Buttons

I haven't done this before and I am trying to figure it out, but I am stuck and I need your help. I set up an ASP page for a survey type webpage. I have groups of radio buttons on this page.

My survey is rated 1-9, I set my radio buttons values accordingly

Example: radio button 1 (value=1), radio button 2 (value=2), etc.

Now it keeps saying it "Could not find form field(s)", I am in the Insert record dialogue box at this point.

What do I set the submit as button too.

Wouldn't radio buttons be "numeric"?

But it is not working, please help.

Never mind on the last question. My brain temporarily quit working. I thought maybe if I explained this better, you or someone else could help.

Where I am stuck is at here: I am in the Insert Record Dialogue Box

The box is laid out like this:

Somehow this part of my posting got left out. Maybe this will make more sense.

The box is laid out like this:

Insert into table _____________

After inserting go, go to ________________ Get Values from ________________________ Form elements ______________________

Column _________________

Submit as ___________________ (this seems to be where my problem is, I think)

I have the option of submitting as:

Text

Numeric

Date

Checkbox Y,N

Checkbox 1,0

Checkbox - 1,0

I don't know what to submit it as. I have tried all of these options, also when I set the insert record box to either of these options I am getting an error saying that the forms fields cannot be form.

The form fields which it isn't finding is all of my group radio buttons. Please help :~(
