No input fields when setting up a server?

I am unable to set up my remote and testing servers. How do I go about fixing this issue so I can finish adding these servers?

Sorry to hear you're having trouble setting up the server in Dreamweaver. Could you please follow the steps mentioned in the link? Unusual behavior in Dreamweaver? Try restoring preferences. and let us know if that helps.

Looks like a bad installation.  Run the CC Cleaner tools and perform a clean re-installation.
