Unsuccessful extension uninstall

I've just used the extension manager (DW8) to uninstall an extension. It's no longer showing up in the extension manager but I can't, even with multiple restarts, get it off of the object toolbar. Any suggestions?

To remove it from the Insert bar, you can go to C:\Documents and
Settings\yourname\Application Data\Macromedia\Dreamweaver
8\Configuration\objects and open insertbar.xml in a text editor like
Notepad. Find the menu and the xml object for the extension in question and
remove it .. carefully .. so you get only that object. Save the file and
reopen DW.

That may not guarantee the extension is gone everywhere .. but it will
remove it from the Insertbar if you think that is the only issue.
