Thank you for a very thorough post... We seldom get one so detailed. It really helps.

Chrome® has its own "proprietary" Flash Player plug-in embedded by Google®. It's called "PepperFlash™" and works on a different acrhitecture than ActiveX™, which is ONLY used by Internet Explorer®, so when you're using Chrome®, you won't see any ActiveX® errors.

Since the PepperFlash™ plug-in cannot be downloaded, updated or repaired from here (Google™ are the only ones who have the plug-in), your best bet would be to uninstall and reinstall Chrome®. Then, if the problem persists, download the  Flash Player™ Plug-in (All other browsers) and install it.

Open Chrome™ and type "chrome://plugins" in the address bar.

In the Plug-ins page, click "Details" at the top right, and scroll down to Adobe Flash Player™ in the list.

You'll see two (2) Flash Players™ listed. Disable the PepperFlash™ plug-in (It's usually the first one listed, but it'll be the PPAPI type)

This will force Chrome® to "fall back" on the NPAPI plug-in you just installed.
