FrameMaker 7.2 on Windows XP non structured

In this one book I'm working on, master page headers and footers show on the monitor. But when I print out a document sometimes in the last few pages the headers and footers are dropped from printing. Making the doccument useless. Some documents in this chapter work OK and in others they may work partially. The first page and a half usually are correct. When I save and update the book with all the boxes are checked and this message comes up "No master pages applied because the master pagemaps in the table contain no mapping information" I'm wondering if there is a relationship between this message and the dropped page numbers and some footers when printing out the document?
I am totally baffled by this quark. As I said, other books print ok.
Backgound Info; I have been importing master formats from books into this pictular book: I've been using FM 7.2 for 8 months and am learning it on my own.
You fine people out there saved me once before and I am very greatful!!!
Could some kind person possibly come to my aid once more.

Jere,

It sounds like you've got the Master Page mapping activated and don't
have the mapping table set up properly. Turn off the mapping on the
Reference pages: go to the MasterPageMaps Reference page and set the
"Book Update" to No.

This is assuming that you aren't actually using this feature to force
special master pages to be applied when certain paratags are being
used.

In addition to Arnis' tip, I have noticed that if I add pages to a document that does not use the default left/right master pages, then the newly added pages are formatted with the default left/right master pages. So, I have to reapply the master pages to the left and right pages for all the pages in the document. Typically this happens for files such as TOCs and Indexes, in which I have set the custom master pages early in the cycle and the files grow as the book gets bigger.
