Reference page technical doc

Not long ago I came across a document that describes all the inner workings of reference pages - structure, syntax, etc. Can't find it again - anyone know about it?

 These pages are used in connection with

• graphics above/below paragraphs
• graphics for reference withing the document
• definitions for creation of TOC and Index
• Definitions for the 'simple HTML conversion'
• Specials by various plug-ins
