TOC Generation

When I try to generate a TOC from a Book file I get the message the book is not self consistent. What does it mean and how can I rectify it?

If you google the phrase, it usually refers to a self-referential xref or link. Try excluding some of the generated files (TOC, IX, etc.) one at a time to isolate the problem file.
