EPUB content out of order

Anyone seeing EPUB files with content out of chronological order?

Also, the TOC is only showing 10% of topics.

Since this is the 3rd edition of this EPUB (My Fm reference book!!!!) I'm tempted to blame this one on Fm 2017 processing.
