I am having trouble figuring out how to change art that was created as CMYK vector art in Adobe Illustrator. The problem is I need to convert the 4 colors (Red, Yellow, Green and Blue to individual PANTONE colors. Currently they are built as CMYK with the screens built as 4/Color Process to make up the red, the yellow, the green, the blue and black tints. The red for example, like the other colors needs to be a PANTONE, NOT a mix of Process colors. The red and the other colors has percentages of all the colors that comprise them along with the black for the shading/shadows, etc. I don't know how to make the blacks (Currently a mix of the Process Colors, into JUST Process Black, either being transparent and/or overprinting over PANTONE colors I need to make the other 4 colors. Got a Hot Job with this one and trying several things, but no luck.

How about a Pantone swatch book, and choose the appropriate (darker) ones, pure Pantone?

You mean changing every tint of the black too? And or the colors? Thank you...

Did you mean to choose just the 4 colors (Red, Yellow, Green and Blue) and change each to match the tints?
Not sure, but If it's the way I think, it would add too many colors.
I'm trying to make it only 5 Colors.

Get a Pantone swatches book and then research the matching colors. THere is no 100% secure way to change this just using Illustrator.
And then make a black gradient and set it to overprint.
If you don't have a printed Pantone swatches book and would like to be lucky, you can "convert" process to spot using Edit > Edit colors > Recolor artwork. There is a button to limit the new colors to the Pantone spectrum. Please read the documentation.
Make sure your color management is set up correctly for the job because the "conversion" depends on it heavily.
It might still go wrong.
