Pen Tool Not Selecting Correct Position

Hi all,

I'm trying to place an anchor with the pen tool, but Illustrator is placing way off from where my cursor is pointed.  In the screenshot, my cursor is hovering in the green circle it (where I actually want the anchor), but when I click, it places the anchor in the purple circle.  It's doing throughout my document, but the distance/angle from my intended anchor is always different.
