Prints not centered printing from Lightroom to Canon PIXMA Pro-200 printer

I've been having significant difficulty getting prints from the lightroom print module to come out centered top to bottom AND left to right. For example, trying to print 8.5x11, canon pro luster paper, 0.25" margin all the way around the photo.

 

Paper is loaded correctly without binding and page size is set right. Prints look spectacular except consistently getting 1-2mm (.02-0.4") differences between margins around the image. Zoom to fit is obviously enabled. Cropping the image beforehand to 8.5x11 did not seem to help. 

 

I've tried:

  1. Page layout Borderless, all margins set to 0, cell size set to 8x10.5"
  2. Page layout Borderless, all margins set to 0.25", cell size set to 8x10.5"
  3. Page layout Normal, all margins set to 0.25"
  4. Page layout Normal, all margins set to 0.5" (testing wider margins)

 

Any ideas/suggestions?
