Since you have photoshop cs4 you should ask in the Photoshop forum:

I'm not exactly clear on what you want the shadow to look like.

In photoswhop elements i made the shadow by using a selection of the shoe, filling a layer behind the shoe with black.
Then Image>Transform>Free Transform to put the shadow below the bottom of the shoe. It looks like that's what you already did.
Then i applied Fil;ter>Blur>Gaussian Blur at around 7.6 and lowered the layer opacity to around 65%.
Then i applied a bit of Filter>Blur>Motion Blur at Angle 0 Distance 57

You shouldn't need a layer mask on the Shadow layer.
Can you post a screenshot showing the shoe plus the layers panel?

Does the orginal image of the shoe have the shoe or a transparent layer or did you have to remove the white background from the shoe image?

What i did was select the shoe then do a layer via copy to put the shoe on it's own new layer, so no layer masks needed. (Layer>New>Layer via Copy or Ctrl+J (windows) or Cmd+J (mac).

i did it more or less as you but no success. I take the original image select subject, copy, new document of 1000x1000, paste in the new document, two layer are created (one with white background and another with the shoe). I create the shadow by duplicate layer and fill it with black, free transform, i obtain the shadow, apply the gausian blur, and again i need to add and remove a layer mask.

The advantage to using photoshop cs4 is you can turn the shadow layer into a smart object and then be able to readjust the transform and blur filters without having to start all over. Double click on the Motion Blur or Gaussian Blur words to readjust.
