My model release has been rejected many times. Please tell me the reason why it was rejected so many times.

The Model Release form is straightforward. Submit form in a legible size in JPG or PDF format. The model should sign the release on the same day as photo shoot. If model is under age, their parent or legal guardian must sign for them and signature must be witnessed by a third party.

I have an account with Adobe Stock Contributor. I had uploaded stock videos of stock models and attached their model releases along with them, but my model releases were rejected several times without giving any reason.

You can upload a copy of the model release here with the model's name, phone numbers and email addresses blurred out. The community members can provide an opinion as to what is missing from the release.

Then I think we need to see the release to determine what might be wrong with it.

Also, I don't think it's allowed to include a link to your website or email address.

Only JPEG is accepted.
PDF is used by Adobe sign, but that will be created by Adobe stock:
