With the Width Tool you can move any part of the stroke to either side of the path at will, so "inside" and "outside" don't mean much. If you need part of a stroke to go farther "inside than the existing path, you can add points and move that part of the path. You can't apply a stroke to the "inside" or "outside" of an open path, either. I hope this makes sense - my eyelids are drooping. Peter ... View more

Mathias, I'm seeing the same problem (even after clearing the appearance), but can solve it by shifting the effects around the Appearance panel at what I think is random before putting them in their proper places. At least I have not yet determined which changes precede proper functioning. I got it to work on your "crappy" example by shifting both the outline and roughen effects a couple of times each, including applying them to the fill before shifting them back to the stroke. If it's an open path, click the starting point (not the ending point) with the pen tool. If it's a closed path, you'll have to cut it. Otherwise, duplicate the brush and reverse ("flip along") its direction and apply it to the path. Or find or write a script to do the job.
