The smart object is still outside the bounds and different proportions from the parent document. I get the same results going back to at least 5 full versions back - this is the way that Liqiufy has always worked with Smart Objects.

Thanks for the file. This is caused by a couple of things in the way file is constructed and the way that Smart Objects work.
I've reconstructed your file (specificall the middle smart object layer) to get the "correct" results where the swirls and bends of the visible trees line up (Please confirm this is what you'd expect - PSD here: https://shared-assets.adobe.com/link/5e257596-bcdf-4de1-67a1-e12e5a642585 😞

The smart object your action creates is actually bigger than the bounds of the parent canvas (I've performed a Image > Reveal All after running your acction to show this):

What @jane-e said, submit the crash report using the dialog in your screen shot but enter the email address in the dialog before you submit it. That will let us search and find the logs for your crash.

I beleive this is working correctly. The Adjustment layer has 'infinite' bounds so it centers the mask as it is the only content on the curves layer. The green layer has outer edge bounds that are just shifted slightly off the bottom and right edges. Photoshop aligns the outer edge of the green pixels to the canvas, and that shifts its mask up and to the left when that layer is centered. The masks shouldn't stay aligned in this example.

Engineering is working on this issue.
Workaround:
Download template from Adobe Stock in a browser:
Enter name of the template in "Find more templates" in New dialog and click Go; or just click Go to go to Adobe Stock and find the template there Download template from Adobe Stock or Save to CC Library
If template was downloaded as .psdt file, File->Open and open it in PS
If it was saved to library, right click on it in Libraries panel > Open New document
In both cases, PS will create new document from this template

It's likely because you have 45 images open. Only 25 images or less (depending on the amount of VRAM in your system) will use the GPU for drawing the document/image. All the rest will use CPU.
