Adobe Acrobat Reader export to Adobe Sign

I have completed a document on Adobe Acrobat Reader and tried to upload it to Adobe Sign.  In doing so, it shows no information I have "filled and signed" from my original document in Adobe Read to Adobe Sign.  The document is completely blank when uploaded to Adobe Sign.  Is there a way to bypass this?

Could you please share the complete workflow you do to fill the form and then uploading it to Adobe Sign?

If possible, please share the screenshots showing the changes.

Share the Reader and OS version on the machine.
