Attaching documents

I'm having trouble attaching documents in a fill in file.  The instructions say to click the paper clip with the file icon on it, but that icon is dimmed and can't be clicked.  How do I get that icon undimmed so I can attach the documents?

Kindly check if the file is not password-protected. Also, provide us with a few more details.

1. Try with some other PDF too is the icon visible?

2. Application version?

3. OS name and version?
