Auto numbering PDF document.

Question

I have a bit of an odd issue that I don't no how to sort out. I have a form within Adobe Reader which is 10 pages long.

I want to print the form 10 times but each form requested a sequential number on each page (so the first print of the 10 page form will have number 21/001 on each page, and the second print of the 10 page document would have number 21/002 on each page) an so on.

can anyone give me some guidance on how to do this easily?

Doing this in Reader is tricky. A script can be used to do it by adding a comment to each print out, but running this script will require installing a script on the local machine that adds a special menu item to the application.

Also, each print out will have to be in a separate command.
