Can't fill in form

I am trying to fill in a form which is password protected. I got into the form and accidentally clicked this pop up that asked me to convert to read only - this pop up has been coming up for MONTHS and I usually just cancel it but this time I just wanted it out of my face and said ok and it covnerted the whole doc to read.

Adobe reader DC is up to date. I can fill in other forms but this one.

Please help me reverse this issue!

    Not familiar with this pop-up. What does it say (please download the form again to get the message again or let us know the web address for it).

    Thank you for your response. The pop up no longer comes up. When I closed, deleted and re downloaded the original form this is what came up:

    Prior to "accepting" whatever it was I accepted, this never happened. I was always able to write on fill in forms. (This form was sent securely with a password.)
