can't open a link

I'm trying to download a form from this website https://hhs.texas.gov/laws-regulations/handbooks/cclpph/forms and I get a message that I need to update my Adobe Reader, but I have the latest version.

Moire than likely you are using a web browser that is not using the Adobe Reader plug-in. The browser specific plug-ins are horrible with forms. To get by that, instead of clicking the link to open the pdf, right-click, save to you computer, open Reader then use File>Open to open the pdf.
