Cannot Save Form with filled & Signed content - error message "Operation Failed"

Every time  I try to save the document/form , once filled and signed, i get the error message below. Then when I click ok it will let me save it but not with the filled in content, that disappears. Can you tell me the solution?

    Sorry for the delay in response. 

    Open the file in Acrobat/Reader, click File>Properties> Security tab. Check options you are allowed to perform on this file.

    Also, check under General tab if the file is created using LiveCycle Designer?

    I am having the same issue.  I was able to successfully use the Fill and Sign tool on my first two docs within the last hour, but now I keep getting the "unknown error" message and cannot save my content and signature on other docs.  Please advise.

    It must have to do with the source of the document.  I am trying to add my signature and text to two invoices from the same vendor.  I just opened a completely different PDF from a different source and I am able to successfully save my signature and text.
