Create form file

HI , im trying to do something but i dunno if its possible , so please give some advice

Wanna create lets say 5 diferent pages with forms so i can imput a name and adress  , but depending the type of work im doing i dont need to auto fill those imputs on all 5 of them , some work i need only to put on page 1,3 and 5 and other only on 1 or 2 for example

Its is possible to create a master so i can click the ones i wanna to fill ?

something like this for front page where i can select the files i wanna work on ;

after that i can imput the data on them.

Dunno if i was clear enought.
