Create pushpin icon for markup

Is there a way to add custom icons to the toolbar? I would like to add a pushpin icon and then i can drop the pushpin on a section of the document.

No. The toolbar can't be customized in Reader, beyond the built-in commands, which can be enabled or disabled only.
