Customers losing data when they save

I make form fillable documents for customers. We are running into an issue with a handful of customers that I cant figure out. Apparently when they save the files the radio buttons they have checked are becoming unchecked. The forms print fine, but when they open a saved form the boxes are unchecked. they have tried this on multiple computers with the same issue.

I cant seem to recreate the issue, and other customers don't seem to have the problem.

Any Ideas?
