From your description, this is usually related to Windows icon cache or file association display issues, not a problem with Adobe Acrobat Reader itself.
The “folder with a 9-square grid” icon typically appears when:
- Windows fails to load the correct icon
- The icon cache gets corrupted
- A recent Windows update or system refresh resets icon mappings
Please try the steps below and see if that works:
1. Restart Windows Explorer
- Press Ctrl + Shift + Esc
- Find Windows Explorer
- Right-click > Restart
2. Reconfirm default app even though you’ve tried this, please try once more via:
- Settings >Apps >Default Apps
- Search for .pdf
- Set it again to Adobe Acrobat Reader
3. Repair Adobe Reader:
Open Adobe Reader, Go to Help > Repair Installation
