digital signature error

Hello,

We are using "request signature" everytime we have a new employee that needs to fill out his application and sign. Everytime after our employee is filling out the document and sign, at the end it shows an error that says "we can't process your request, please reload the page and try again". After this, the employee signs again and then it works. How we can fix this error, so we don't need to reload the page and do the whole process again? I will attach a picture of the error. Thank you!
