Does anyone know how to fill out the Secretary of State form on the Bsportal website?

I'm having trouble filling out the company name change form "540 Certificate" to update your company name, it keeps giving me an error message. Someone told me to save the PDF File first then open Adobe to attach the saved file, I'm so confused on why the Secretary of State website didn't just have this updated on their website.
