Error in displaying fillable document in Reader DC

Question

I have a customer that is using Acrobat Reader DC and whenever they open a fillable form in Reader, their document will display the attached image; it is normal prior to attempting to fill it out. The document behaves normally when you open it in a browser (Chrome in this case) and it does this for all fillable forms.

I have uninstalled Reader, run the AcroCleaner 64 bit tool, and reinstalled with the latest Reader DC installer, but the behavior is persisting. I was wondering if anyone has run across this issue before as I am not sure what to call it to do a search.
