Error occurred when attempting to Fill and Sign

Hello.

Everytime I attempt to use Fill and Sign, the Error Report box appear. And after the report is sent (or don't sent), the adobe acrobat windows is closed automatically. I've tried uninstall and reinstall Adobe Acrobat, but the problem still remain.
