Error when I print a pdf fillable form

I have completed a pdf fillable from from a website I use at work every day.  When I print it, it prints the previously saved pdf fillable form.  I believe I have clicked something somewhere to cause this error but how can I correct this?  I hope this makes sense.

    Please check for any information on the PDF form under the form fields. Also, share the screenshot of the print settings window. 

    If it is an issue with a particular file, would you mind sharing the PDF form with us so we can look into this?
