Fillable PDF Form not printing in correct location

Hello,

We have a PDF Fillable Form we use internally in our office. We complete the necessary fields and then print onto specific paper that has the corresponding blank fields (along with other text).  When I used to use this fillable form on our Windows PC it would print with no issues, however, now I am using a new Mac Mini and when I print, the fillable fields no longer print in the correct spot on our template.

I am using the most current version of Adobe Reader, I have also updated my iOS to the most current, and installed all the correct drivers for the printer (and done all troubleshooting with the printer manufactuer to rule out a printer issue).

Any assistance would be greatly appreciated as this is something I use pretty much on a daily basis. If any further info or clarification would help, please let me know!
