Fillable PDF with checkbox but also sum a score total

I am trying to make a PDF where I need to two things to happen-for context it is a symptom screening tool.

I needs to have the ability to checkmark a response and I would like it to tabulate a total score.   For sample there is a question posed and 4 answers to pick from; the answers are add to the over all score on the screen tool.  I hope this question makes sense
