Form in 3 different languages

Hi! Experts,

I have a form that i use for collect info for my business.
i have 3 different forms for 3 different languages.
Is possible to have just one form with 3 flags on the top ( British, German, Spanish ) and selecting the flag automatically change the language of the form?
