How do I fill in fields on a dynamic stamp?

I had a dynamic stamp created which contains 4 text inputs. I'm confused on how to fill them in.

A dynamic stamp is "fixed" once it is applied. Typically text fields would be filled in via some Acrobat JavaScript as part of the "launch sequence" of the dynamic stamp.
