I have prepared a form in Adobe, however, when I request e-signature it alters the formating

Hello,

I have created an adobe form through the prepare a form option. When I add the unique text to the fields and select the appropriate check boxes everything looks good on my end (before picture added). However, after the last adobe update, when I then go to e-sign the request e-signature, all the formatting changes. Allcheck boxes now default to unchecked and the formatting no longer uses the multiline option (even though its selected) so text run outside text box. This form has been used for years and I completely rebuilt the form to make sure it was something within the old form. Please help!
