Sorry for the inconvenience caused. As per the description above, you are not able to send the form using the Email button in Adobe Reader, Is that correct?

May I know, which application is used to create the form?

You may try to send any other PDF form as an email using the Adobe Reader DC email button. You may refer to Attach to email option is not working | Acrobat DC, Acrobat Reader DC
