Information stuck on PDF file

Every time I create a PDF, somehow, a Patient's name has gotten permanently stuck on every PDF file that I have made since. I am not sure how it got there and I don't know how to remove it. I need it gone, please advise.

    How did you create the PDF files? Acrobat Reader can't create PDF files.
