Lines of table are distorting

I have a template which i use for my business and recently if I use the fill n sign feature, it seems to distort the lines of the table. The lines become squiggly I guess for lack of a better term.
Also, sometimes as I type my text, the line of the table above the text begins to disappear as I type.

This is a recent issue and it worked perfectly well up until about 6-7 months ago.

Thank you

Actually, I just renamed the doc and it works now.

    Thanks

    Hi Puckfan,

We're sorry for the trouble you had while filling a PDF form using the Fill and Sign tool, please reboot the machine once and check for any pending updates of Adobe Reader DC from the help menu>select check for updates.

Reboot the machine again after updating Adobe Reader.

Would it be possible to post a screenshot of the issue you are experiencing while filling the PDF form? To post a screenshot, take help from the article How to share screenshot

Are you on a Windows machine or Mac and what is the version?

Have you tried using the Fill & Sign tool with a different PDF form and checked?

If it is specific to one PDF form, try downloading the file again from the source and check.

Is it possible to share the file with us so that we can test it at our end? Share the file in the private message only How Do I Send  Private Message | Adobe Community

    Thanks,

Ok, I tried a different pdf doc and it seemed to work fine with the fill and sign feature.
I am guessing the file I was originally trying to use may have become corrupt? I have used the same blank doc about 200 times.

    Using Win 10

    Adobe is up to date.
I am using a saved copy over and over for my customers. I originally created the pdf in Open Office.
How would I send it to you?
Tried the method you recommended but it does not seem to work for me.

    thank you
