Lots of problems with this form

This came to me via email to be filled in and returned. 1) the box that persists and says to wait for the form to complete (got advice and got rid of it); 2) the form will not allow me to sign as the icon is grey and I can't change it; 3) it will not accept the completed form to return in an email although I have upgraded as prompted. This is my will, rather important and I'm really fed up with the form.

I would really appreciate advice to get this properly handled and off my desk.

I have a MacBook Pro, running Yosemite 10.10. and MS Mail.

    I hope you already have found a solution to this problem. Looking at the issue description it is hard to comment on it.
