messing up my document

I complete forms required for a new job and after signing and saving adobe snt me the complete copy and has completely messed up all my information please help

We need much more info than that to be able to help you... Starting with:

- What application did you use? What's your OS version?

- What do you mean by "Adobe sent me the complete copy"?

- What exactly happened to it? In what way was it "messed up"?
